Dr. Gago,

After how long of unsuccessful trying is it appropriate to seek additional help with trying to conceive?

We have a 3 1/2 year old son that was conceived after 12 months of temping & charting. I was diagnosed with endometriosis (stage 1) shortly after my son was born. My past medical history also includes a surgery to repair my ovary due to a large cyst.

Over the past 3 years, we have 17 cycles that we have tried, but they weren't consecutive - 8 months on, 6 months off, etc. As of right now, we are on month 8 without success. My OB/GYN said it will be hard to get pregnant, but not impossible. I met with him in February and he has suggested the use of Clomid, but doesn't want me to start it until August/September.

I have been told ever since I was 15 when I had my ovary surgery that I may have trouble getting pregant and I have tried to be patient, but I'm ready to get pregnant! What would you suggest? Wait until Aug/Sept for clomid, or take the next step now?

How many years young are you now?

In general, after a total of 12 months of attempting conception without success, whether consecutive or not, an evaluation and appropriate treatment is warranted.

If a woman is over 35 years, then 6 months is enough time to wait before starting an evaluation/treatment strategy. Women over 40 should have an earlier evaluation to make sure that any treatable factors are addressed.

Having previously diagnosed endometriosis puts you at risk for an earlier decrease in your ovarian reserve. Getting started now would make sense.
